Samwell killed whole armies of Others, fighting for 40 days and 40 nights. Then he went and had 40 more days and nights of straight wild, crazy love-making with Gilly.
Only then did he break his fast by baking every single Frey into a pie and then eating them.
Or at least, a thousand years from now, that's what the songs will say.

It's never made sense to me, but maybe he needs insulation for the Winter that is Coming?

There's a point where he notices he's lost weight, I think. But as Hurley on LOST said, "For the record, my belt HAS dropped a notch. I'm a big guy. It's gonna be a while before you're going to want to give me a piggy back ride."

Therefore I would like to put in a good word for him and state that even at his "prime" Samwell was never as fat as lord Manderly.
And after all his struggles and being reborn as Sam the Slayer, I'm sure his belt has finally stopped slipping now.
His father Randyll Tarly is described as lean, so there is hope for poor Samwell!

Also, is it just me, or are there more men with excess fluff in these books than there are ladies? There's Fat Walda and Lysa, and their fluffiness doesn't seem to be morbid, nor is it described in quite as gross terms as Sam/Manderly/Ilyrio/YellowWhales's is.
I always pictured Fat Walda as rather cute. And Lysa just as sort of unhappily puffy.

I love Fat Walda! It seems to me that in ASOIAF the most unattractive trait a woman could have is being skinny or hairy. Selyse gets way more descriptive rancor than Fat Walda or even Lysa.

Roose is totally her bitch now.
But it's true, women aren't getting that much. Well I suppose you could add Genna Lannister-Frey to this list but she isn't described negatively either. Something like once beautiful but got big cause of having kids and age or something IIRC...?

Sam is still a boy, give him time to get into his stride... I'm sure after a nice soujurn in Oldtown he'll start putting those capons and pork pies away again like a champ.
Randyl Tarly is lean because he's been kicking ass 24/7 since the day he was born, basically he is a medevial Chuck Norris.

"Genna Lannister had been a shapely woman in her youth, always threatening to overflow her bodice. Now the only shape she had was square. Her face was broad and smooth, her neck a thick pink pillar, her bosom enormous. She carried enough flesh to make two of her husband."

9/10's of Samwell Tarley's body weight is, in fact, being carried in his fat pink mast.
This explains everything-- his utter immunity to diets, the irrational hatred of his own father (jealous of Samwell's imposing size), his landing of a doe eyed, nubile, buxom young wilding beauty who excites the lusts of Dolores Ed and others (and you just thought this was yet another moment of authorial vicarious living and/ or standard trope about the fat nerd getting the girl!)
In the end, it is simple-- Samwell Tarley= roughly 150 pounds
Samwell's mast= 100+ pounds.
