After how long can I be totally sure that my topic was not approved? Also, since there is rule of moderators not telling you what is wrong with your topic, would it be ok if i continued to post improved versions until i figure out what was wrong with my topic? Of course I would (and did) check rules to try to figure it out; I am not just planning on spamming same version over and over. If that is not the right course of action after topic does not get approved, what would be more appropriate?

If posting a topic in a topic-approval forum, here's the advice:
First, check that your topic is appropriate for the forum. The Winds of Winter forum, for example, is specifically for discussion of the upcoming book - topics not specifically related to that book should go elsewhere.
Second, check that your topic is not already covered by an existing topic. By far the majority of threads that don't get approved, are rejected for this reason.
Third, wait to see if it is approved - this takes a varying amount of time depending on which forum it is (not all of the moderators go into the current TV series forum, for example, because not all of us have seen it yet), how busy that forum is, how many moderators are available, etc. Generally, though, if it's more than a couple of days, the topic is unlikely to be approved.
I really can't see many reasons to 'improve' or modify a topic that isn't approved. If it wasn't approved, it was likely for a good reason: almost certainly because there's an existing topic that covers it. You should post in there. There are no bennies or buttons handed out for starting new topics. Nevertheless, if you feel you must, do it once and then leave it at that - a topic that's been rejected twice and keeps popping up in the queue is, with the best will in the world on our part, not likely to receive a warm welcome from the mods.
