To me the following acts of heroism come at great personal cost and what is more important, the persons who went through with them didn't have to do them. They all could have stand aside but chose to act because they believe it was the right thing to do:

Ned protecting Jon

Ned speaking against murdering Dany to the King and council

Ned trying to protect Cersei's children (as fool as it was is admirable how he was willing to overlook that it was the Lannisters who crippled and tried to murder his own son because he couldn't stand the murder of children. Too bad his heroism was wasted in the wrong people)

Davos sending away Edric Storm

Syrio Forel protecting Arya

Sansa saving Dontos

Jon letting the wildings through the wall

Qhorin Halfhand and Squire Dalbridge giving up their lives "for the realm"

Jaime jumping on the Bear pit to save Brienne

Jaime killing Aerys

Brienne saving those children at the inn

Catelyn going against a man armed with Valyrian blade to protect her son

Qhorin wasn't putting everything on the line: he was sacrificing his life for a greater goal (in a situation where he didn't have much choice). What if, instead of (or perhaps in addition to) sacrificing his life, Qhorin had abandoned the greater goal to make a moral stand? In that situation he truly would be risking everything: not just his life, but everything he'd ever worked for.

That's not heroism, sorry, that's stupidity. If your morals do great harm, perhaps it's a good time to reconsider them.

OK, let's take Davos. What does more harm: sabotaging your own war effort, or allowing the death of a little boy? Davos had to make that choice, and he decided that as much as he wanted Stannis to win, he thought saving the boy's life was more important.

Jon as Ned's bastard was a possible threat  to Catelyn's children's claim to Winterfell.  Jon the son of Rhaegar was a threat to the lives of Catelyn's children, not to mention the lives of Ned, Catelyn and anyone else Robert thought betrayed him.  Considering how fiercely Catelyn protected and defended her children, I think she would have hated and resented Jon even more if she knew the truth (assuming blah blah blah) and hated Ned for it too.  So I think Ned was wise in keeping that fatal secret to himself.

I think Meera and Jojen were brave to go all the way to the CotF with Bran.

It was a horrible move tactically and wound up really wrenching Robb's war plans, but I've always thought, misguided as it was, Edmure Tully's bringing all of the smallfolk into Riverrun while under the prospect of a Lannister siege, and then riding out to meet Tywin Lannister, one of the most seasoned and feared men in the Seven Kingdoms, to protect his home was incredibly heroic.

Another one is Tyrion riding out in the Battle of the Blackwater after Sandor Clegane deserts, because they're is no one else to do it. He knows he's probably going to die, but he does it anyways. Pretty cool in my books.

Tempted to say Tyrion saving Aegon, just cause the kid is such a prat and Tyrion had absolutely no reason to risk his life for him, yet did.

But Tyrion leading the flank on Stannis' forces attacking the wall comes to mind just because of the sheer epicness of it in the series. "Half-man! Half-man! Half-man!"

Objectively I'm leaning towards Jaime saving Brienne in the bear pit though. Absolutely no reason for him to do it, she wasn't family, wasn't really a friend, wasn't any monetary gain to him doing it. Yet he leaps into the bear pit without a sword, or a sword-hand for that matter. Pretty impressive.
