I have to agree with Darth Rivers, his command structure is far more servicable, maybe with the exception of the outriders.

Overall commander/reserve - me
Outriders - Blackfish
Vanguard/right - Robert.
Rear guard/left - Ned
Center - Bittersteel
Commander of cavalry in my bataille - Daemon Blackfyre. A walking (riding) morale boost where it is most needed.
Commander of the footmen in my bataille - Blackheart. Competent professional soldier.
Commander of archers in my bataille - the guy from the Golden Company
Lord Admiral - Stannis
Warden of the west - Jaime Lannister post Harrenhal. The guy started growing a brain there, and he isn't a walking morale disaster like his brother.
Warden of the east - Bronze Yohn
Warden of the south - Tarly. Gigantic asshole, but competent soldier
Warden of the north - Robb Stark

army of the dead

surprised no one has put Anguy for captain of archers

He crossed my mind as he is possibly the best archer in the realm but he isn't a commander.  He would be better suited serving as a captain

Leader of scouts - Brynden Tully
Commander of van guard - Gregor Clegane (I hate him but he can lead reckless charges and I wouldn't mind sacrificing him)
Commander of rear guard - Jon Arryn (he's smart enough to be able to maintain order in the ranks if there's a rout)
Captain of archers - Jon Snow (he did well when he held The Wall against the wildlings)
Cavalry general - Loras Tyrell
Commander of the left - Eddard Stark
Commander of the right - Robb Stark
Commander of the centre - Robert Baratheon
Commander of the reserves - Tywin Lannister
Admiral of the fleet - Davos Seaworth
Second in command - Stannis Baratheon
Commander in chief - Aegon the Conqueror
Generals seven bodygaurds - Arthur Dayne, Barristan Selmy, Oberyn Martell, Jaime Lannister, Addam Marbrand, Balon Swann, Jon Umber

Westerosi only (or permanent resident of Westeros in the case of Areo Hotah). No "historical figures" (so no Dunk, Bloodraven or Baelor Breakspear). Assuming everyone is in their prime:

Leader of scouts- Oberyn Martell
Commander of van guard- Robert Baratheon
Commander of rear guard- Tywin Lannister
Captain of archers- Beric Dondarrion
Cavalry general- Robb Stark
Commander of the left- Garlan Tyrell
Commander of the right- Jaime Lannister
Commander of the centre- Randyll Tarly
Commander of the reserves- Jon Snow
Admiral of the fleet- Stannis Baratheon
Second in command- the Blackfish, commander in the field
Commander in chief- council of Eddard Stark, Doran Martell, Tyrion Lannister, Jon Arryn, Asha Greyjoy, Willas Tyrell, and Howland Reed.
Generals seven bodygaurds- Loras Tyrell, Brienne of Tarth, Arthur Dayne, Barristan the Bold, Areo Hotah, Sandor Clegane, Greatjon Umber

Only problem with a council of leaders is no decision maker. Would just end up with huge amounts if arguing and no plan

Cavalry general- Loras/Garlen Tyrell

Commander of the left- Addam Marbrand.

(I'm sure we all feel this way but) I definitely picked the best team!

Leader of scouts- I want a pair: Jojen and Meera Reed
Commander of van guard- Gregor Clegane. Scare 'em good!
Commander of rear guard- Brienne of Tarth. She's loyal, but I wouldn't put her anywhere in front.
Captain of archers- Jon Snow. He's not as good as Anguy but he seems to be a good teacher to others.
Cavalry general- Khal Drogo can ride better than anyone yet mentioned.
Commander of the left- Blackfish. Probably one of the smartest commanders in the series.
Commander of the right- Mance Rayder. He hits with full power.
Commander of the centre- Ser Loras Tyrell. Stay close to Renly <3<3
Commander of the reserves- Lord Renly Baratheon. He's charismatic, so the reserves will be psychologically ready.
Admiral of the fleet- Victarion Greyjoy, duh.
Second in command- Prince Oberyn Martell.
Commander in chief- Danaerys Stormborn.
Generals seven bodygaurds- Sandor Clegane, Barristan Selmy, Strong Belwas, Visyrion, Rhaegon, Drogon, and Arya Stark. No one gets through.

Yeah, I'm pretty sure that's an unstoppable army.

Using Darth Rivers's command structure due to it being the correct way (I like the OP's breakdown but I'm trying this one too)

Overall Commander- Lord Eddard "the Ned" Stark
Overall Commander's Honor Guard- (the Honor guard has no limit as opposed to the KG
(LC) Ser Gerold "the White Bull" Hightower
Ser Barristan "the Bold" Selmy
Ser Aegor "Bittersteel" Rivers
Ser Archibald Yronwood
Areo Hotah
SmallJon Umber
Sandor "the Hound" Clegane
Anguy the Archer
Ser Oswell Whent
Ser Arys Oakheart
Ser Balon Swann
Vanguard/Right- Lord Robert "the Demon of the Trident" Baratheon
Rear guard/Left- Prince Baelor "Breakspear" Targaryen
Reserve- Ser Arthur "the Sword of the Morning" Dayne
Commander of the Footmen in my Bataille- Ser Daemon Blackfyre
Commander of the Archers in my Bataille- Brynden "Bloodraven" Rivers
Command of the Cavalry in my Bataille- Ser Brynden "the Blackfish" Tully
Captains of the Outriders- Ser Addam Marbrand
Lord Admiral- Lord Stannis "the Mannis" Baratheon (with Ser Davos Seaworth and Victarion Greyjoy as Second-in-Command)
Warden of the West- Lord Tywin Lannister (if Tywin has a reason to be loyal to you then he's the best choice)
Warden of the East- Lord Jon Arryn
Wardens of the South- Lord Randyll Tarly and Lord Garlan "the Gallant" Tyrell
Wardens of the North- Benjen Stark and Robb Stark
