Will we learn anything about the first Kingsguard-how it was decided, the members' houses and background, essentially the origins, in the World of Ice and Fire?

I think if GRRM includes the first members of the king's small council then he may include the first members of the KG.

Yes, the World of Ice and Fire explains in detail who the first Kingsguard were, and why and how they were selected. It's good stuff.

Yeah that sounds fascinating! It would be nice if we got to know more about the Greatheart aswell as about the other legendary KG members that we dont know much about atm. But then again knowing "just" who formed the very first KG is more then enough on its own!

We're aiming for Fall 2013, artists allowing. A lot of sketches rolling in right now, though, so it seems to be going well on that end.
