It's always about Jaime; don't you know, all the ladies are 'in Harrenhal with him..?!' Likely she pushed her into the well when she was least expecting it, the same way Littlefinger caught Lysa, who like Melara implicitly trusted her murderer, unawares. There's a parallel between pushing someone into a well and through the Moon Door, into the 'bloody blue' of the water and sky respectively! Regarding hints; there's also this one: The shade of Melara haunts her and has 'accusing' eyes. Why would her eyes be accusing if Cersei were not guilty of drowning her? LoL yes "all the ladies" yes, true! I had forgotten Cersei's Ghosts!

Whether Cersei pushed Melara or simply refused to go get help when Melara fell in, Cersei killed Melara. There is no scenario where Cersei is not responsible for Melara's death. BalerionTheCat
Maggy told Melara she would die this night. But she didn't try to prevent it. Why would she? IMO it was the first step of Cersei doom. Because Cersei killed her herself. Had Melara lived, maybe Cersei prophecy or curse would have been voided, because Maggy would have been already wrong.
