The "Ser" title is knighthood. Kevan and Lancel were both Knights weren't they?

Kevan for sure at least, and Lancel was a Squire but raised up I believe after Robert died. 

And yeah I think Lord Varys was just a formality. Normally to get that title you need to own lands.
