Essos needs Daario, Jorah and Grey Worm on there I reckon.

Crownlands is I guess where one would place Thoros of Myr.

Jon Snow and Tormund need to be mentioned too, somewhere.

Randyll Tarly in the Reach.

Bronn - who knows where to put him. Crownlands I guess?

Right now in the North the best is probably Hosteen.

In the RL, it's probably the hound.

In the crownlands, I have to go with Gregor.

In the stormlands, it's probably someone from the GC on their way to SE.

In the Reach, it's probably Garlan or Andrik.

In Dorne, Areo most likely.

In the Vale, Lyn Corbray has the best reputation but I'm not sold.

In Dorne, I think Daemon Sand and Ser Gerold Dayne can probably match up with/be included in a top 10 list of anyone left alive after ADWD.  I also think Ser Archibald Yronwood and Areo Hotah are formidable.  

Jason Mallister should be included along with Brynden Tully amongst river lords.  Mallister seems to have a good tourny rep and backed that up with impressive showings on the Trident and the Greyjoy rebellion in true combat situations.  However, both may be a little old to be truly elite after ADWD (especially Brynden).  

I think some of the people listed we don't really have proof for.  Randyll Tarly for instance - he's probably one of the finest commanders in Westeros; but, there's no proof that he's someone you'd want to choose in a Trial by Combat.  Similarly, I feel like Bronze Yohn is probably not in his prime any more/a little too old to be considered elite.  

Andrik the Unsmiling is noted as the best in the Isles.  Victarion, Ser Herras, and Qarl the Maid are probably the next best.  

In the Stormlands, Ser Richard Horpe is a candidate.

Nah.  Ser Pounce, like any True Knight, is too chivalrous to fight a Lady.  It was still he who caught that villainous mouse.

I wonder how he blazons his coat of arms?

I imagine a rampaging feline standing over the corpse of a slain mouse on a field of red - similar to the Lannister arms but with a cat instead of a lion.

You can use the lance in actual combat not just jousting.

Indeed. However, you it is an impractical weapon in close combat. Should the lance survive the first contact with the enemy it would likely be discarded for an alternate weapon.

For the river lands it should be Ser Hosteen Frey instead of Black Walder. In the novels Hosteen is mentioned quite a few times to be a fierce fighter, and also the best warrior out of all the Freys.
