I don’t have the quote on me, but I do recall reading that he used the Ptolemys as inspiration for Targaryen incest.

Well, in the middle East (Islamic countries, I mean) more than half of legal marriages (around 55%) happens between second cousins and closer relatives (people aren't allowed to marry a niece/nephew/uncle/aunt, making a marriage between first cousins the most incestous). It's all about how much DNA you share with the person you'll have a child with, and how much bad mutual DNA you share. The more you share of the bad ones, the higher the risks. Since these people been doing it for so long (we speak of common usual people here) they are slowly leaving behind the bad ones due to natural selection, making the risks smaller and smaller. It doesn't make them hideous, etc. 

As of Charles of Spain, it is important to know that altough his parents were cousins, the two shared (slightly) more DNA than siblings would. Because of the many previous incestuous unions which they came from in a short time. And Charles of Spain was, even considering that, and exceptionally unlucky case.

Back on topic of Baseless Predictions:

- Duncan the Tall had 5 children, 3 boys and 2 girls, same as Egg.

- When Dany will be brought to Vaes Dothrak, amongst dosh khaleen she will meet a woman who is Khal Drogo's great-grandmother and Viserys Plumm's daughter. She will tell Dany a story that her father Viserys, same as Dany's brother Viserys, also sold her to the Dothraki. Same as Dany she was married to a Dothraki Khal, though as a price for her that time the Dothraki gave to the newly formed Golden Company 10.000 horses. While in Dany's case her price was deemed by them to be higher - they were willing to give to Viserys not only 10.000 of their horses, but also 10.000 of their warriors.

- When Dany will arrive to Westeros, Brienne will meet her mother - Pretty Meris from Windblown, because Tattered Prince will join forces with Dany's other armies/companies. And then thru Meris GRRM will reveal to readers that Brienne is Dunk's descendant, and that his old shield was left at the Tarth's armory by Meris, prior she left Westeros and her newborn daughter.

- Jon Connington will go to the Citadel, where he will meet Sam. And afterwards af a payment for Sam curring him, he will escort Sam back to The Wall, where he will meet Jon and will realise who Jon is. Because Jon's personality is exactly like Rhaegar's. That's why JonCon is already suspecting that fAegon is not Rhaegar's son - his personality is totally different from Rhaegar's.

- TWOW will be released in 2023, and ADWD in 2025 (at least I hope so ^_^).

@Jaenara Belarysit makes sense for the Targaryens because they wanted as much Valyrian blood as possible in their descendants veins to control dragons. This is achieved by incest because there are no other Valyrians to breed with.

Starks were also practicing inbreeding to keep their blood as pure as possible, because their Pact with the Children was based on blood magic. For the ritual that sealed the Pact was used the blood of Bran the Builder, the founder of House Stark. That's why there always has to be a Stark at Winterfell, furthermore that Stark has to have at least some of the Last Hero's genes.

Look at their family tree (if you have read my theory written in the 16th part of the Swan Song) - all Starks nearly always married not only with Northerners, but specifically with those Northerners to whom they were bloodrelated. Aside from Alysanne Blackwood - Cregan Stark's second wife.

Out of Cregan Stark's ten children - 1. Rickon, 2. Sarra, 3. Alys, 4. Raya, 5. Mariah, 6. Jonnel, 7. Edric, 8. Lyanna, 9. Barthogan, 10. Brandon. - 

the daughters of 1 (Sansa and Serena) married with 6 & 7 (their half-uncles),

a daughter of 2 married with a son of 10

a daughter of 3 married with 10 (with a guy who was her half-uncle),

a great-granddaughter of 4 (Arya Flint) married with a grandson of 10,

a granddaughter of 5 (Melantha Blackwood) married with a grandson of 10,

6&7 married with the daughters of their older half-brother 1 (Rickon).

The mother of Cregan's children 6-10 was also a Stark - his third wife, Lynara Stark.

So as you can see - Starks favoured marriages with people who also were carriers of Stark blood, same as them.

Same as Targaryens the Starks were keeping their blood pure, because of magic-related reasons. Targaryens had to follow that "delightful custom" out of necessity. And unlike the Starks, after the Tragedy at Summerhall, Targaryens didn't had a vast pool from which they could have chosen other carriers of the same blood as theirs. Starks were doing the same thing as Targaryens, only in their case it was less obvious, because they had more relatives from whom they were able to choose spouses.

To my favourites:

Arya and fArya will meet in Bravoos, fArya will take Arya´s place with the FM so she can head back to Westeros without a bunch of of revenge-seeking Assassins on her trail.

Already mentioned here, Arya will mercy-kill Stoneheart. Also believe that she will break her Needle doing so and will later get Dark Sister as replacement which Bran found in Bloodraven/Brynden River´s cave.

Septa Lemore is Ashara Dayne, Jon´s mum, Aegon/Young Griff is Lyanna and Rheagar´s child.

My utterly baseless prediction, mainly in the expectation that GRRM will lean even more into horror territory than he has already:

  • Cersei is (or will be) pregnant with a boy, but it will be stillborn just as the White Walkers reach King's landing. The child will then be risen as a wight and strangle Cersei, fulfilling Maggy the Frog's valonqar prophecy.
