It means 1) Bran is starting to remember how he came to fall from that tower, and who pushed him; and 2) the crow has three eyes, and the place where the three-eyed crow is pecking is on Bran's forehead, where his "third eye", or rather, sixth sense is a better way of looking at it, is "closed".
