The exception is when the woman is the heir, in which case she keeps her last name, and the children take hers. Arianne will still be called Martell when she gets married. If a woman's birth house is more important than her husband's house, she'll be called by her maiden name more, like Cersei Lannister. Offical documents and formal court introductions probably call her Cersei Baratheon, but in conversation everyone calls her a Lannnister because that's the primary identity for her.
