Rhodan
I am not sure if people in Pentos or Braavos are puting given names first and surnames second or if they have it vice versa asian/hungarian style. After all Ilyrio Mopatis is constantly refered as "Magister Ilyrio" and Syrio Forel was "Master Syrio" (I think) but , of course, that might be just how the culture is dealing with given names. Darth Rivers
I am pretty sure those are given names first. Afterall, you have "Lord Tywin", "King Robb" and etc. in Westeros, where we know the given name is first.
The Imp With A Pimp Limp
I think they're all first names. There's also Khal Drogo and his name is Drogo. Then like said above people refer to Jaime as Ser Jaime and so on
Arya Nymeria Stark
I think a lot of it is "title, first name" because especially in Westeros, everyone knows who someone is, like King Robb, everyone know's he's Robb Stark. Also, Jaime Lannister is referred to as Ser Jaime cuz he's a knight of the King's Guard.
