Melisandre seemed to worry about getting burned by her ruby during the Rattleshirt execution, so it appears that high level red priests do not become completely fire immune, though they have massive cold resistance modifers (reference: see Melisandre in the North/at the Wall). She seems to have changed her internal chemistry in a way that the internal heat overrides the influence of external temperatures.
