- Tyrion having Symond the singer killed

- Tywin supporting Janos Slynt as NW LC despite knowing what kind of shitty commander he is

- Tywin neglecting his children and their happiness

- Tywin giving up on Jaime as his son as soon as Jaime is captured

- Joffrey attacking innocent butcher boy and lying about whole incident

- Cersei betraying Falyse Stokeworth

- Cersei (probably) killing Melara

- Cersei letting countless dwarfes be killed and not punishing their murderers (for fear they would hesitate in they saw real Tyrion)

- Cersei not standing up to Sansa when Joffrey abused her (despite her resentment of Robert's abuse)

- Joffrey threatening to rape Sansa at her and Tyrion's wedding

- Tywin sacking KL (really, why was this necessary? He only needed to defeat Aerys, not sack the city)

- Tywin keeping Gregor Clegane around despite knowing what horrible and monstrous man he is

- Cersei wanting Arya killed (after Joffrey and Mycah incident)

- Joffrey humiliating Tyrion at PW with no good reason

- Joffrey killing citizens of KL with crossbow

- Tywin having Masha Heddle killed (owner of the inn when Tyrion was captured)

- Tyrion objectifying women in general

Sending Janos Slynt to the NW under the orders to kill Jon Snow

Tyrion sends Slynt to the wall for carrying out the order of a murder of a baby. Later, Cersei tries to send a Kettleblack to the Wall to assassinate Jon Snow, but he never ends up leaving.

- Tyrion for not turning in Podrick for murdering Mandon Moore

One is not confirmed, and Podrick didn't murder Mandon Moore, he protected Tyrion from a murder attempt.
