Biter the Gallant
Well, a Crakehall and a Swyft could be left with the Lannister to defend CR, and would thus have all been able to have contact with the prisoner. I think the same, just HelenaAndTheMachine suggested that perhaps all off them were prisoners. And surely members of the main Western houses periodically have to appear in CR on special occasions anyway, to express loyalty etc. like the many Northern nobles whom visited Bran at the harvest feast. These guys could even be some wards, who knows.
Biter the Gallant
Based on the ConCarolina reading this week it is unlikely to have been a Reyne. I meant that to be a joke, but what did Martin say exactly? All the Reynes are surely killed? That destroys the "Littlefinger is a secret Reyne" crackpot also :-)

That destroys the "Littlefinger is a secret Reyne" crackpot also :-) We can explain, but perhaps reading the notes of the reading will answer all questions that arise about stuff we forget to mention :) What bacially happened to the Reynes, is that their castle was 9/10th undergrounds, and they had fled in there, expecting to be able to witstand a siege for about 3 years, IIRC. Tywin did not agree with their tactic (softly speaking), and had the mines flooded, after effectively blocking off all the other exits. The people inside all drowned. All other Reynes had previously fallen in battle.
The Lady Lannister
I cannot do spoliers on my phone. Sorry. But it is strongly implied that no prisoners were taken. The notes are on the History of Westeros blog.
