You don't need to be running around thinking you're a dragon or locking up your sisters to be mentally ill. Cersei is deeply paranoid and exhibits a lot of sociopathic behaviour. Not only has she been cruel and vindictive from a young age, but she only seems to be able to love those that in some way embody or resemble herself. All of this however, you could argue is someone who has the bad combination of not being very empathetic, and a lot of unchecked power.
