History has plenty of examples of inbreeding between families in ruling dinasties. The most typical example are the Egyptians Ptolemies who married within the family to keep the bloodliens pure (as the Targaryens). The famous Cleopatra of Egipt was the offspring of a brother-sister relationship. If that were not enough, his only set of grandparents were uncle and niece. And still. Cleopatra was perfectly sane and considered a skilled ruler at the time.

According to wikipedia's article about incest, in an union between sibilings "only" about a 20-26% of their children will die or have major disability due to the inbreeding.
