Things in the series with the Ice and Fire concept?

I've seen a number of people say that in the series there are things that have to do with the name of the series, A Song of Ice and Fire?

For example there are two battles about to start at the end of ADWD, The Battle of Ice and The Battle of Fire, Ice and Fire. People have also pointed out that Jon is of Ice and Fire as well, being Lyanna's and Rhaegar's son, and also the dragons and The Others being the opposites of each other and each being said to be of made Ice and Fire.

Are there any more things like this that give both Ice and Fire it's name?

Rh'llor (god of fire) and Great Other (apparently connected with icy Others)

In the vision in HOTU, Rhaegar says He is the Prince who was promised. His is the song of ice and fire.

Again in HOTU - blue rose growing on the wall of Ice, is somehow connected with Dany's (fire) future
