It's mentioned that he suffers from such internal pain and migraines that he quaffs milk of the poppy like it was wine. He's an individual that lived his entire life in horrendous, mind-shattering pain. I suppose if there's a good quality to Gregor Clegane, it's that he paid for his actions every day of his life, or that he was a victim of his condition.
