The visions may have gotten crueler, driving him mad. Aerys wasn't always mad, kind of a douche in his early years, but in his later years he gets obsessed with fire. He may have seen fiery sacrifices in his dreams or something. We know from Dany's chapters in GoT that the visions can really be scary. When Dany first sees a dragon in her dreams, he describes the dragon's eyes as pits of hell. Aerys probably got a load of these and, unable to understand the visions, went mad.
