Fire and blood magic

Fire and blood seems to refer to Valyrian magic process:

1) We know that Daenerys hatched dragon eggs by giving them "fire and blood": execution by burning, with eggs within flame.

2) Valyrian steel swords are likely made the same way, with combination of dragon flame and blood sacrifice.

Are there any other examples of this type of magic at work?

The Valyrians didn't have to pull a trick out of a fire like that.  Their eggs were hatching normally.  Sacrifice was not required to hatch the eggs.  You may be right about the making of the sword.  Which would explain why the Targaryens stopped commissioning for new swords.

The Weirwoods seem to represent a similar magical path as dragons. 

With blood sacrifices and burnt offerings being found in their maws. 

One might also consider eggs and acorns to be symbolically related. 

It could be that the Song of Earth is the source of the Valyrian ability to manipulate stone.

GRRM regularly seems to indicate opposites by placing "And" between two concepts.  

So the phrase Fire and Blood may be synonymous with the phrase Fire and Ice. 
Perhaps also indicating a connection to the God of Flame and Shadow.
