But as it is, not big enough to be a threat to a kingdom. If you kill the sparrows etc or move on them, you are basically declaring war on an entire faith, you open yourself up to be mobbed and easily justified for ambitious lords to denounce you and have a "righteous" reason the public can get behind to destroy you. All in all they probably have a couple hundred actual fighters. We notice through the POV's that many are smallfolk and women looking for something better. I doubt they could effectively win a war, but ransack King's Landing? Sow seeds of doubt? Easily. The high septon when Aegon landed decided that the faith and Oldtown had to side with him or else they would burn. Large parts are untouched, and many parts did not fare as badly as others. It's not like every singel soldier in the region is dead and all it's smallfolk gone. The Frey's still have 2,000 in Winterfell and 200 in Riverrun, 250 Tully men were allowed to leave Riverrun unharmed, the Brackens had a force at Raventree until recently and the Blackwoods had a garrison, Vance of Atranta, vance of Wayfarer's Rest, Piper, Smallwood, Lychester,and more had token forces at Riverrun... Also, the majority of the 3,500 men who were massacred at the Twins were northmen, not rivermen. Sure, several thousand of their men have died, the best of them at that, but many still remain. When they were talking about cutting Cersei free there was no doubt that they could do it. It was just that they didn't want to be seen as the type of people who would kill septons and such.
