My beef with the ACA, aside from my continuing (but now irrelevant) objection to the mandate, has been that it doubles down on the ad hoc structure. I don't disagree and even share your frustration, but unfortunately a redesign of the system was in 2009, and remains in 2015, a political impossibility. The main problem is that one of the two major parties doesn't really think the health care system ever had a problem, except that too many people were suing it. We already have enough people in other professions who won't do their jobs (like pharmacists who won't fill birth control) and often get away with it when they should actually be looking for a new line of work that doesn't interfere with their belief system. And talking about a terretory the size of the US it should be obvious that you are dealing with a lot of different situations. Thats why the founding fathers thought it would be a great idea to organize the US as a federal system and not a centralist state. That's three state's rights right there. Although I'm not clear on why they should have had the right to force states to do something when they found the idea they that might be forced to do something so unappealing they seceded and fired the first shots. Vermont's gun death rate is much higher than neighboring states with tighter gun control. It's overall suicide rate is also higher than neighboring states. He's not wrong that there's a difference place to place in how guns kill people, but the solution is simple and the same across the board- fewer guns, fewer deaths.
