For sheer blood/violence/splatter/rape content, the winner would be American Psycho, followed closely by Stephen Donaldson's The Real Story. Honorary mention to Shakespeare's Titus Andronicus. For bleakness and grinding sense of futility, H.P. Lovecraft, and Orwell's Nineteen Eighty Four. For morbid decadence, Clark Ashton Smith's short stories.
